Nevada's diabetes rate exceeds the national average with over six percent of the population estimated to have diabetes.  In 2002, HAWC joined the Bureau of Primary Health Care's Health Disparities Collaborative focusing on diabetes.  This Collaborative brings together health center teams for twelve months and under the guidance of national care experts, they promote rapid improvement in care through a dynamic Chronic Care Model.  In 2003, HAWC graduated from the Phase I and Successfully passed into Phase II of this highly acclaimed national program.
