As a service to our patients, HAWC has an on-site pharmacy to
provide low-cost prescription medications. Patients referred
from HAWC's Outreach Medical Clinic are also able to receive
medications at no cost. Even HAWC's senior patients on
Medicare benefit from HAWC's low cost medications. Through
this service, HAWC is able to ensure that underserved individuals
and families receive adequate care and vital follow-through with
necessary medications.

SHARING THE CARE
A PHARMACEUTICAL ACCESS PROGRAM
PFIZER PHARMACEUTICAL
Sharing The Care is an ongoing program that provides needed medicine
to low-income, uninsured patients free of charge. Through this
program, eligible patients are able to obtain most of Pfizer's
latest, state-of-the-art prescription medicines directly and
immediately. The HAWC Community Health Center participates in
this program.

To be eligible for Sharing The Care, a
patient must meet all of the following criteria:
1. Have an annual income at or below the federal poverty
level.
2.
Not have any private insurance or public assistance covering
pharmaceuticals.
3. Not be Medicaid enrolled, even if pharmaceuticals are not
covered.
4.
Be a registered patient of the HAWC Community Health Center.
A patient who qualifies for the Minimum on HAWC's Sliding Fee
Program is automatically eligible for the Sharing The Care Program.
Patients are advised to bring the following documents on the first
day of their visit to apply for the program:
1. Identification Card or Driver's License of the head of
household.
2. Proof of Address
(Any utility bill with the head of the household's name)
3. Proof of Dependents
(Social Security Cards or shot records of dependents)
4. Proof of Income for the past 12 months.
(Income Tax Return for the last year or pay stubs for the last 3
months)
